I buy almost all my fish from a lfs in Pekin IL called Charlie's Aquaruim supply. There is a Charlie..altho his son runs it now. He goes on buying trips up to Chicago to get his fish, and I find them well cared for, a good variety, and the store staff very knowledgable. The others I've picked up at our fish club's auction. I have bought betta's from PetCo, because they keep their bettas in bigger cups than all the rest of the pet stores.
I get supplies where I can...Petsmart, Big AL's, ebay..plants too. Charlie doesn't have squat for plants.
 
Mostly I buy from my fish from a good little local store. It's called The Menagerie in Toronto. Mostly has smaller, unique fish like dwarf cichlids, rainbowfish, barbs, livebearers, shrimp, and all kinds of other oddballs.
 
You know another great place to pick up fish.. the local bait shop, some times you can get some really cool native minnows there. So if you are ever into building a Native tank with out the Tank busters try the local bait shop.
 
I go to one of the two LFS's in Abilene when I can, but they just don't have the stock that Petsmart has. I've gone by Wal-Mart for a few things, too.

I generally order online from importers or travel abroad to collect specimens myself.
