Why did my angel die?!

I have a 50 gallon corner tank that has been set up since late November. It was cycled when I added fish. It currently has 3 angelfish, 6 corycats, a beta, and 2 redfinned tetras. My numbers are:
ph 7
ammonia 0
nitrite 0
nitrate 15
temp 76

I have not seen any aggresion within the tank. An occasional chase, but no fighting. I did a water change Monday, and all was well. It has been a busy week. Friday I did a headcount, and could not find one of my favorite angels. Finally found her dead. She had been dead a while. So I could not really tell anything from her body. I did have three ottos in there about six weeks ago, two are gone. The remaining one I moved to a tank with lots of algea, and he is great. (Maybe lonely, but has plenty to eat.) I did lose one corycat shortly after adding some about six weeks ago. I assume he was not healthy to begin with. I have a souple of low light plants, fake plants, and some other decorations. I have not changed any decorations recently. I use conditioner for the water. Not prime, haven't gotten that brand yet. I use the same buckets for water changes. I cannot think of anything that caused her to die. Thanks for any feedback.

On I think an unrelated not, I had an issue with ick in two other tanks. They have been sucessfully treated with advice from this site. And my angel tank has shown no signs of ick. Thanks again.

I have not seen any pairing of any angels before or after. And I really have not seen aggression. Of course if it just started I could have missed it. Last week was very busy for us. I have been watching this weekend closely and have not seen anything.

It is tap water so I assume it is the same chemistry or close. I also did water changes on my other tanks at teh same time. I will test my tap water and see what that says. I adjust the temp of the tap water to close to the tank water. It is not exactly the same, but pretty close.
 
My reading form the tap are:
ph 6.6
ammonia just barely above 0
nitrite 0
nitrate 0
I forgot to add that I use a liquid test kit from AP.
 
They are from the same store. The angelfish and beta were added first and all at the same time. Then I added three corys, then later three more. Actually the first additions were the two tetras(they came with the tank), and a rainbow shark(he came from another store a year ago). He was there a couple of weeks in the beginning until my 55 was ready. He did occasionally chase another fish if they came to close to his cave, but he never caught them. He just moved them along then wnet back inside his cave. He has not been in the tank since the middle of December.
 
which angel died (or is the pic of survivors)?

oto's and angels is not a good mix..angels will attempt to eat oto's

the water sounds good. tho I would bump the temp up a bit as 76 is a bit on the low side for angels.(IMHO)

the angel could have been sick when you got it. did it show any signs of stress?
prior to it dying?
 
These angels are the survivors. The one that died was silver with the black stripes. I looked a long time to find one with those markings. I am going to miss her.

There are no longer any otos with the angels. Two dissapeared, so I removed the third. He is fine even if a little lonely.

The remaining angels and other fish all are very active. They eat well, and seem to be great. I have had them a couple of months, and thought they were healthy. But I am still pretty new to fishkeeping. Thanks for the advice, I will bump the temps up a bit.
