Worms??!!

I have a 125g tank with 1 oscar and 2 convicts. Recently I noticed tiny little white worms (about 1-1.5mm) crawling on the size of the tank on the glass. I have restricted feeding because there are some planaria as well but the ones on the glass don't seem to go away. I have sand as the substrate and all the waste tend to float on the sand though I try to suck them out with a vacuum once or twice a week with WCs. Do you know what kind of worms this is and how to get rid of them.
 
2 filters: Rena xP3 & Aqua Clear 110
Ammonia: 0 ppm
Nitrite: 0 ppm
Nitrate: 20 ppm

I do WCs twice a week, roughly 15-20% each time.
 
Sound like planaria or nematodes. Harmless to your fish. If you want to get rid of them buy some prazipro and give your tank a treatment according to the directions on the canister. If it is one of these two, the prazi should kill them without harming your fish or plants.
 
Thanks, I will look into getting some prazipro although generally I'm not too keen on putting chemicals into my tanks.
 
Usually I am antichemical as well but prazi is non toxic to fish and can't be overdosed so don't worry too much about it. I use it as part of my QT process in anyways to take out any flukes which sometimes can be asymptomatic.
