Do you have a pic of the sword? They grow from a central crown (leaves up, roots down) that should not be buried.
Old leaves (or the emersed form) often die back where the leaf stem meets the crown. The leaf may stay green for a bit but if the stem is dead you should just remove the leaf all the way to the crown. Look for new growth in the center, the leaf stems (petiole) are usually shorter on the submersed leaf form.
You can use small stones to hold down the roots if they're too short or too few to keep the plant down.
