Fish stuck in bubble ladder...Every happen to anyone else?

I woke up this afternoon (I work nights), and went to go turn on the aquarium lights and saw one of my neons stuck in the bubble ladder! I have this one http://www.drsfostersmith.com/product/prod_display.cfm?c=3578+3747+8981&pcatid=8981. It was stuck in the column on the right hand side up against the glass. I thought he was dead but then I saw that he still had all of his color so I yanked the ladder out as fast as I could. Luckily he seems fine. He swam away quickly and is schooling with the others. I can't even tell which one is was now. My question is, how do I prevent this from happening again? I can not think of a way to block the column off so that a fish can get in there without stopping the way it functions.

Keep a close watch on him. I had a zebra danio get sucked up my python and thrown onto my front porch. (A little more extreme, I know, but he seemed fine upon returning him to the tank, a few days later he was dead). My betta in the 3g actually lets his body get stuck on my filter intake. Everytime I see it I think he's dead, but he is literally just chilling out. Off topic, but the thread reminded me of it.
 
I use cut up pieces of foam from a replacement aqua clear filter to block off things to stop fish from getting in. I use the largest sized foam for inside the power filters just behind the outflow as it has the larger pores and doesn't get blocked up with debris. My Pleco and Botias just love getting into things.
 
Hes still alive so I would assume that he hadn't been stuck for very long. For now, I just took out the whole ladder until I can get something to block off the column with.

It's weird, but a lot of fish that do really stupid things, get stupidly lucky and seem to survive. I had a small batch of baby tiger barbs a while ago, and one day two of them disappeared. i thought they got eaten... until i went to clean my HOB filter and found them happily swimming in the bottom of it. i returned them to the tank and they grew up just fine.

Fish are amazing little creatures. When i had my first tank i used to take all my fish out, put them in a bucket, COMPLETLY clean the tank, fill with fresh water, and then throw them back in. I lost a few fish (and learned alot) but a few of those fish lasted me till they were too big and had to be sold/donated to new homes.
 
yeah i use to have guppies swim in behind the bubble ladder diffuser all the time to pick off stuff in there to eat. the ladder seems to like to have a lot of things settle on the rungs. but the guppies always found a way out somehow - off topic too but my betta likes to get sucked up to the sponge filter and chill too! lol
